Q11) My spouse had
her period delayed by 2-3 days. As we are concerned, she took
a home pregnancy test. It was a positive. Since it was an unplanned
pregnancy, we were concerned and just before we decided to go
to a dcotor she had the period. Does that mean that the home pregnancy
test was wrong? Is it possible that even after having periods,
there are chances of pregnancy. Please tell us the next step we
have to take.
Srinivas,
A. 11 The
home pregnancy test has 5 % false positives results also. She
night has had a very early pregnancy or bio chemical pregnancy
if your wife had periods. Still she needs to repeat pregnancy
test, if positive to sea a gynecologist

Q12)
We are married for two years two months. We are planning for kids
for 8 months now without luck. We consulted a Gynic lady doctor
in Bangalore and she performed tests for both of us and confirmed
that nothing is wrong after seeing our reports. She advised us
to be patient and keep trying. She prescribed Fertomid tablets
for my wife and asked her to use them for three months. After
three months, my wife still could not conceive. Can you please
let us know why this is happening? We are not able to bear this
delay in having kids. Please advise.
SSK
A12. Nearly
80 % of couples conceive with in two years of un protected intercourse.
As you have been trying for 8 months and your wife is below 25
years of age you can still try for 6 months before you sea a doctor
for help. She should take clomiphene only under supervision of
a gynecologist it is better to be seen by a local gynecologist.
You have not mentioned about semen analysis. If it is not being
done you can get it done, as it is a basic and simplest test.
If it is normal and if she has regular cycles you can try few
more months between 13th to 18th days of cycle I.e. that is the
time of ovulate on and has better chance of conception.

Q13) I am 30 years old and got
married 4 years back. Still I do not have children. Daily I am
enjoying sex with my wife. Why my wife is not getting pregnancy.
We both satisfy with sex. I want children. So, please give your
advise.
suresh
A.13. You
have not mentioned about your wife’s age but as you have
been trying normally for the last 4 years it is better to consent
a local gynecologist who might help you in getting basic investigations
done and advice some simple treatment. In the mean while before
you see your doctor you can try normally alternate days between
13th to 18th days of her cycle if her cycles are regular.

Q14) My wife is 8 weeks pregnant.
Last week she got herpes. Our gynaecologist asked us to consult
a dermetologist and he adviced to take ACIVIR-800 DT 4 per day
for 8 days. We are also applying SOFRADEX in the affected area
(Around the waist part). Since the area got some fuss, he prescribed
CLARIBID-250(8 tabs, 2 per day). We are worried taking so many
tablets will affect the pregnancy badly? She had a miscarriage
last year in 2.5 months pregnancy. Please advice as soon as possible
to take some corrective action. Your immediate response will be
greatly appreciated. Thanks very much..
Raj
A.14. As you
mentioned that your Wife had previous of miscarriage it is difficult
to say whether it was due to a similar problem during the previous
pregnancy. The management of herpes during early pregnancy is
very difficult as untreated herpetic infection also increases
the risk of miscarriages and fetal problems. Normally any medication
before 12 weeks of pregnancy can affect the fetus but as most
of the organ genesis is completed by 8 weeks and she has also
has a high risk with out treatment. So she can continue the medication.

Q15) I
need ur help. I am married since 2 years and 7 months ago I have
delivered a baby boy. Now my breast have become very soft and
small also. Is there any exercises to make them stiff and big.
Is there any use of creams available in the market..
Rajani
A.15. The
changes what you had abut the delivery is quite physiological.
You might see your doctor for exercises, which might help in improving
firmness. Creams like masse cream used during antenatal and postnatal
period may help but it may not during this time. If you are really
worried a cosmetologist may be helpful.

Q16)
thanks for doing this wonderful job. I got married 1 year back
and we both don’t want to have childern for atleast 2 more
years. Recently my wife got abortion, my question is when we want
children after 2 years, is there any side effects at that time
,as because my wife got abortion .I would appreciate if you give
answer to this..
SAS
A. 16. You
have not mentioned about your wife’s age, which is important
for conception, & you have not mentioned whet her it was spontaneous
miscarriage or whether you got terminated. The chances of conception
in the initial years of marriage are higher and that is the period
when woman’s fertility is better. Slowly trying the chances
of conception come down and other problem may arise. So I personally
would advise to try for a conception with out too much of a gap.
If she had an evacuation after miscarriage there is a small risk
of having a tubal problem.

I AM 30 YEARS OLD AND I GOT MARRIED 4 YEARS BACK.STILL WE DONT
HAVE CHILDRENS.DAILY WE ARE ENJOYING THE SEX.MY WIFE CONSULT WITH
LADY DOCTOR.SHE TOLD EVERYTHING IS RIGHT.I DIDN'T MET ANY DOCTOR.IWANT
YOUR HELP IN THIS MATTER.WHICH TEST I HAVE TO CHECK UP.PLEASE
ADVISE ME SIR,IAM WAITING FOR YOUR REPLY SIR.THANK YOU VERY MUCH
SIR.
Suresh
A.17. As your
wife’s tests are all normal you can also go through some
routine basic investigations. Semen analysis’s to check
for sperm count and motility is an import out test to be done.
If your test is also normal, your wife can go through ovulation
induction and you can try for conception during ovulation days.
If is better that you consult your doctor and take his advise
regarding these tests and treatments.

Q18)
I am 26 years old got married 11 months back, i have got irregular
periods from long back(nearly 8 years) when i consulted gynecologist
she gave me birth control pills at that time for 3 months. After
using them i used to get regularly but after few months again
problem use to repeat. It happened nearly 3 times using these
years. Right now I am in US and went to gynocologist they tested
for hormones and conformed I am having hypothyroid and my physician
suggested Synthyroid pill daily one. My gynecologist also suggested
me to take birth control pill until my thyroid gets balanced.
My question is will I get pregnant bcoz I am having hypothyriod
and it is lately detected.
I am also having another problem that is, I am having a small
bubble(2 mm in size) with a small hole(sudhi mona size) under
it on top of my vagina. I dont feel any pain or any other problem.
Can u please tell me what can be this.I will be very thank full
to u if u answer these questions. Thanking u Sir.
Madhavi
A.18. Regarding your skin problem. You may be having vesicle or
it could be hair follicle infection. It is better that you are
seen by a dermatologist or a gynecologist who can tell you precisely.
You might be having polycystic ovaries, which causes hormonal
imbalance and irregularity in ovulation and hens’ irregular
cycles. Once your thyroid hormone levels are under control, has
no impact on conception As soon as your thyroid levels are normal
if you are planning for a child should consult your gynecologist.
You may need to try tablets like clomid, which are ovulation on
inducing drugs.

Q19) I'm 28 years old. I got
married 3 years back. I got pregnancy in July 2002.i saw the heart
beat on 8 week but on the 10th week I have under gone a miscarriage.
So after 1 year we planned again I got pregnancy(July 2003). I
met doctor on 8th week I saw heart beat...
I asked doctor that I want to see heart beat on 10th week also...i
took appointment...again I went for check up on 10th week of my
pregnancy ...that day doctor told me that she is not able to seen
heart beat. At any time miscarriage may happen.... after 3 days
started spotting. Then I was admitted in the hospital they did
DNC. Me and my husband took some test (hormonal, chromoges etc...)
all are normal for me and my husband but they did test for prolactal...it
was 27.4%(hyper normal).....will this cause any problem if i plan
for preganancy again.can i plan for preganacy now?....what all
precautions do i need to take if i get preganancy again. I'm working....i'll
travel in bus daily morning 1hr and evening 1hr....can i take
car for office in preganancy to avoid bus. Please advise me as
soon as possible
Nikitha
A.19. It is unfortunate that you have miscarriage twice during
the same period. As most of your hormonal reports are normal,
you have good 70% of chance. Having pregnancy to continue. So
don’t get dis hearted and if you are medically and physically
fit. You can try for conception. Your prolactin levels are not
too high, and some times anxiety also can raise the levels we
normally advise for a repeat test and if it is still high, your
doctor may prescribe Bromo Criptine tablets for you normally travel
by bus or car is not contra indicated except if you have backache
and spotting during pregnancy.

A. 20. You
do have chances of conception naturally and other wise with treatment
also. all the treatments have limited success rates, and none
is 100%. IUI has a success rate of only 10 – 15 % per cycle,
but being simple’s treatment one can try Up to 5 –
6 cycles by that time 40 % of couples conceive. If these treatment
do not work IUF \ ICSI would be the next option which gives 30
– 40 % success rate per cycle. Since you have tried only
2 cycles. You can still try few more cycles in U.S. your gynoecia
doctor would be the best person to suggest you regarding to which
infertility clinic you can go.

Namaste. My mother's age is 44 years. She got menopause 2 years
back. She got periods now. She undergone all tests. Reports are
normal. Doctor suggested her Hysterrectomy operation. Is that
operation really required? Sometimes she is having white discharge.
Is there any solution without operation? what is ur advice? Please
give ur advice as early as possible
-saritha
Saritha
A.21. You
did not mention if there is any cause found for her postmenopausal
bleeding. If she had only one episode of bleeding and there are
no. Other causes found, she can wait before she goes for hysterectomy.
If there is any indication of uterine polyp, an abnormal pap smear
of any evidence of malignancy hysterectomy is essential. But her
gynecologist is the best person to advise her..

Q22)
Periods are not coming since last five years. Present age 50 yrs.
Conceiving whether possible?
Lakshmi
A.22. It is
very unlikely for a pregnancy to happen at your age group and
especially if your periods are stopped five years ago. But if
you are worried you can get serum FSH done and if the levels are
high, it indicates that you are already reached menopause.

I am 28 years old. I have Polycysticovarian disease. I have been
married for 4 years. I have been taking the treatment from past
two years and during this period I have had two miscarriages within
first 55 DAYS. I WOULD LIKE TO KNOW WHAT IS THE REASON, IS THERE
ANYTHING
DOCTORS CAN DO TO AVOID SUCH. A PROBLEM.
Veena
A.23. High
CH levels which are found in poly cystic ovarian disease can be
one of the reasons for miscarriage, if no other causes are found
for your previous miscarriages. Medication like using Metformin
may help in reducing LH levels. A surgical procedure like laparoscopic
ovarian drilling also is one of the measures to reduce LH levels.
Your doctor would be the right person to suggest if any of these
treatments are going to be of any use to you.

Q24)
I am married since 3years but without child, my wife periods are
normal almost everything is fine her side according to doctor,
but my semen analysis say’s sperm count is nil, doctor suggested
us to go for ICSI then, in order to that they did testicular biopsy
and same time they try to retrieve sperm from testiest. Unfortunately
they were no sperm.
So we could not go for ICSI.
Testis Biopsy Histopathology reports details as follows:
MICROSCOPEIC DESCRIPTION
The seminiferous tubules on both sides show spermatogenesis but
hardly any mature sperms.
The basement membrane is slightly thickened.
DIAGNOSIS:
Testes, biopsis – Spermatogenic arrest.
Recently I happened to some articles in the web about male treatment
like Male stimulation (to grow sperm) and hormone treatment. Then
I met one doctor who was saying specializing in this treatment.
And I was told that treatment will be three months and the expensive
part is very high (daily around Rs.1800/-).
My
hormone Test details:
FSH 10.7
LH 10.8
PRL 76
T (Total Testosterone) 4.30
SHGB 36.9
Would u please suggest me on this and should I go for this treatment
any alternatives. Really I am very upset about my future life.
An
A.24. As your
FSH & LH levels are mildly elevated and testicular biopsy
has shown spermatogenic arrest, the chances of improvement in
the sperm count in spite of taking hormonal treatment is very
less. But if you are not tried any treatment. Psychologically
you may feel the treatments would have helped you. So as you have
no other options except to for donor insemination. You can take
a chance and try with the hormonal treatment.
